I have had quite a few people with mixed results from PRP. Plantar fascia degeneration is normal so ultrasound is not really diagnostic as the pain generator. It would be important for you to know if it nerve, muscle (flexor digitorum brevis, abductor hallucis, or abductor digit minimi) or fat pad atrophy prior to making the decision.
I have a friend that is a PT that diagnosed it. Went to an orthopedic 3 weeks ago that confirmed diagnosis. They took X-rays to rule out bone spurs. Pain when walking, running, biking. No pain when no weight on foot. It started last August. I was able to manage pain and run 45-50 miles a week until 5 weeks ago. Did a hard threshold run on treadmill (progression to 5:30 pace) and it flared up bad later that day. Couldn’t run for two weeks. That’s what prompted visit to orthopod.
As far as determining nerve, muscle, or fat pad. Is that only done by MRI?
