The right side (rear derailleur) shifter is missing the plastic cap on the side and ring that allows you to switch between index and friction shifting (Dura-Ace 9-speed). I have a call into Shimano, but haven't heard back yet. My LBS couldn't help me. Can anyone help?

go onto shimano website and go to technical documents. Get those part number, and give them to your LBS to special order. Probably the easiest way.

Did you look under the couch? My missing parts always end up under the couch
I have a mis-matched set in my parts bin. Left shifter looks to be 8 speed, right is a pretty new looking 9 speed. They are missing the brackets that mount them to the bars. They would be good for parts if you can't find what you are after. $20 gets 'em.
