How are these custom one piece aerobar extensions typically mounted? Are they threaded so just one bolt through the bottom? Or is there a hole through the entire riser stack so they're clamped from bottom and top?

It depends on the basebar. On mine there's actually thread in the basebar, so I just put a bolt through the extensions and spacers into the basebar.
I have a gen 2 speed concept (Monoriser). Speeco made a piece to replace the mono riser and add their custom bars. FastTT made a plate to replace the mono extension. Anyone could get the plate and put any aerobar on it. I bought a pair of FastTT bars. Look up their website, they have guides you can see hoe to attach their bars to pretty much all of the bikes out there. So you need the company to be making the part for your bike to add them on, which is what got in my way until last year.
