Any recs on where to stay? Better to stay a short walk from the swim start or one of the hotels with race day shuttle? How did the race day shuttle work out in the past? Is swimming distance off the beach easy mostly everywhere or tricky?

If you can swing the $$ Cozumel palace is well placed. Hot corner and close to finish. We always stay at Blue Angel (for dive or race trips) a mile down the road
You want to be near finish. Plenty of transport to T1 for the bus to the swim
There are a few places to OW swim safely, but not a ton. Not very many “in town”. From the car ferry to Villa Blanca pier is a good spot. Just watch out for boat traffic as there is a lot

We stayed at the Grand Park Royal. It's walking distance to town and there are plenty of shuttles to the swim start. You can swim right off the beach. If you need a longer stretch of water there are places between the hotel and town. There were plenty of taxis after the race to take you back to the hotel.
El Cantil condos. Best spot on the island.
