Shoes: No improvement over specialized trivent for me: might be dependent on pedaling style as someone I have tested at the tunnel actually did save Watts. But I checked twice, no gain for me. I raced with the trivents, as the toebox is so narrow in those s-works that I can only stand them for 90 min of riding.
Top tube bottle: I cannot reach bottles behind me, but wanted another bottle. Small aeropenalty for me that I gladly take. Unfortunately the setup started to come loose during the race.
Oversized pulleys: No idea. Did not test with regular pulleys.

Yes easily done on a road bike. But I will leave it to someone else to try that on a TT bike in the aerobars.
You need to set the cranks at 90/270 for best results. Save another 1 watt.
In all seriousness, could you actually get into that position while riding?

Every season during a slow point in the early season races, some pro will do it to get a little camera time for sponsors and break up the monotony.

i think they are called Lezyne side flow, you get them as left or right sided. On amazon.
