never put on your wetsuit for the first time the day of your big race.

never switch gel types for the big race.

never change any components on your bike for the big race.

Here's one I learned watching my 12 year old daughter set up her transition area during her first tri:

Never leave for the swim start without first untying the double knots in your running shoe laces

Never put on your rubber undies and stuff them full of sardines before going out to cut the lawn.

Never grab an IM athlete's bike by the saddle after the bike portion of the IM and then run your fingers through your hair...

Do NOT miss your wave start because you are fretting about your transition set-up.

Don't arrive 10 mins before the start and expect everybody to part like the Red Sea on the bike racks to make room for you.

Don't start at the front of the swim unless you also have a better than 90% chance of finishing the swim leg FOP. Let the fishies go.

Don't forget your bike helmet and/or front wheel. I've seen both happen at pretty much every race I've ever been at. You can get by w/o most of your other gear if need be, but not these 2 things (assumes ya didn't forget yer bike too)

Eat a banana 5 minutes before the swim start, board the boat while gripping your stomach, refuse to jump in the water because it is a 5 foot drop, get pushed off the boat by the lifeguard, freak out and throw up your pre-race oatmeal and fresh banana so that everyone behind you has to jump into your puke.

never take the day off before the race, especially if you have to drive 4+ hours getting out to the hotel/motel/holiday-inn the day/night before.

Never ever ever only just drink water on the bike portion of an IM! I learned that the hard way in my first IM attempt (DNF due to dehydration)

...assume that your tummy will settle down and you don't need a pre-race poop after all! Hasn't happened to me yet, but I've been tempted to skip it before. Results could be nasty.
