Help diagnose my hip/groin pain

Background: December 1st I was running, day after doing a lot of lunges. Made a quick side step to avoid something, felt pain in my hip flexor and groin. Jogged home and iced it. Didn't run again for a month. Next time I ran, I felt great for ten minutes then all of a sudden I got pain in the crease of my leg that ran down my hip flexor into my groin. I stopped and iced. Tried again a month later, ten minutes felt fine then the same pain. Rinse and repeat three more times.

Finally, I went to see the doctor. Got xray and mri. PA said I have a labral tear in my hip and I definitely need surgery. I then went to a specialist who regularly treats professional athletes. He said I have a very slight labral tear and it was so slight that I may have even had it for years and it has been asymptomatic. He said try PT and hold off on surgery. I opted to get a cortisone shot and try PT. That was three weeks ago.

PT is going very well. I am making good progress. I am getting into running with a 2 min run/ 3 min walk interval. First for 30 minutes, now at 35 minutes. My last one felt ok, I was a little tight in my groin after, but I was able to work out the tightness with a foam roller. Now here is the interesting thing. I have also been working in cycling, which has made my leg feel GREAT since I first got injured. It was on a trainer all winter, so I figured that once I got on the roads, cycling too would inflame the injury. I live in a neighborhood with very steep hills and thought that would make matters worse. But, the more I cycle, even on the steepest of hills, the better and looser my leg feels. I just biked yesterday and today and I couldn't feel better.

My question is, what type of injury is it where cycling (down in the drops and dancing on the pedals on steep hills) would make my injury feel great, but running for a prolonged period of time would lead to tightness and eventual groin pain? My thought was stress fracture (impact related), but that didn't turn up on the MRI or xray, or at least wasn't spotted. Plus, I also felt some tightness and pain at certain points when I swam and kicked hard.

One other clue was that the PT on my first visit said I had an incredibly tight illiapsoas on the injured side. He worked on it quite a bit and has me stretching that thing every chance I get. Thanks for your help and input.

Similar story.

The run is aggravating it likely because of posture. You are likely not activating glutes and collapsing in the middle ( bending at the waist) so the pelvis is tilting the wrong way.....you then get impingement.

Run taller, lead with your belly button and not your chest or chin. Tilt your pelvis and try to get up on your toes.

I still get hip pain but stretching and concentrating on form has improved it greatly.

Thank you! I noticed that I was doing that the other night on the treadmill and told myself to run tall, as you say. I bet it stems from putting my head down to grind my way up the hills in my neighborhood. So no surgery for you?

So far no. I spent about 8 weekly sessions with a great pt. Plus got back to better form and things have improved. I still need ibuprofen occasionally. I am actually running faster than five years ago.

Defenitely don't look at feet. Hills is the best drill for proper posture. Look 10m on front of you.
