I do it just like I would for a race: Shift down to a lower gear about a mile or so from the end of the bike to spin the legs a bit & then slip out of my bike shoes with about a 1/4 - 1/2 mile to go. Hop off, slip on my running shoes and then straight to the run. No rest in between the bike & run. I take the first mile of the run to get my running legs under me and get to my race pace.
I normally walk for a 1/2 mi. after I run to cool down and to stretch a bit.
