'Statement of Intent' WTC qualifying question

On the eagleman site it says that if you're hoping to qualify for Kona or Clearwater that you have to fill out the 'enclosed statement of intent' form. Just where is this enclosed form? I clicked yes to the boxes on Lin-Mark regarding the qualification about 7 months ago when I signed up, is that what they're referring to?

All entrants must complete the enclosed Ironman "Statement of Intent". You must be present at the Eagleman 70.3 awards ceremony to claim your slot, and then must sign the Ironman Qualifier Certificate. You must then submit the entire 2006 race entry fee of $475 ( US), plus the GTG One day Ins Fee of $10 for a total of $485.00. Please make check payable to "CTA." The Ford Ironman Triathlon World Championship will be held in Kailua Kona, Hawaii on Saturday, October 21, 2006. If the athlete withdraws on or before August 31, 2006, $210.00 (USD) of the entry fee will be refunded in lieu of $205.00. Applications for the Ironman lottery are available on the Ironman website. Athletes who try for the lottery and do not make it will need to complete the full and correct Ironman application if you win a slot at the EagleMan 70.3, and send it in prior to the due date.
