To me, it looks like the fork wasn't properly preloaded before the stem was installed.
If you were to grab the bike at the fork, could you 'wiggle' it relative to the frame?

If so, try:
Loosening the stem clamp from the fork steerer tube.
Tighten the stem cap to preload the fork and frame interface (you may need to add a small spacer here).
Re-tighten the stem clamp to the fork steerer tube.
