Anyone have any experience with this injury, surgery, recovery?
I’ve had both peroneal longus and brevis surgeries. I was in a cast and on crutches for ~2 weeks, then a boot several more. I started riding the stationery bike in the boot. Once I got the splint, I started doing elliptical. You will definitely need to be thorough with following your PT regimen once you start running again. Here’s a previous post about my experience:

Actually, I’m having that surgery and a groove reconstruction in 2 weeks. It seem like if you are just getting the tendons repaired, recovery is not too bad, 3-6 weeks. If it involves modifying the bone and making a groove then recovery is much longer (and more painful).
This is all just conjecture, but I have become an expert on the peroneals since these issues have started occurring 2 years ago.
How was this injury diagnosed? I am having difficulty with pain in my peroneus longus and have been getting different treatments with no real progress.
Yes, I have had it - first time by an orthopedic sports medicine surgeon. Lasted until he took my boot off in his office - I stepped down the curb - and the tendon immediately slid back over my ankle bone. He then sent me to a podiatrist. She did the repair, regrooved the bone and made a retinaculum. Boot for a week, then a cast for 4 weeks (I think) and then the boot for another 4 weeks. Lasted about 6 months - then the tendon completely severed while I was walking. So I was offered a cadaver tendon - No thanks! This has been about 2 years ago. I completed IM AZ this past year - did fine until about mile 6 on the run & felt a sharp pain in the opposite ankle. Finished up by walking. I had major swelling and pain the next day - MRI 3 days later showed peroneus tear. I wore a boot for a couple of weeks. I have elected not to have surgery this time. I am now “running” again & it seems to be doing okay.
Just make sure you pick a skilled doctor to do the surgery.
Had surgery Dec. 23. I was in a boot for 12 days did some easy spinning and swimming starting 4 weeks post op. Have not tried to run yet. The foot is still a little sore but more on the instep than the ankle. Also some paresthesia of the last two toes and the sensation as if my sock were slightly bunched up under those toes. The ankle started to get sore two years ago… no single precipitating event however the ankle does have a history. Twice when I was young I injured it playing Volleyball. I didn’t break it but might as well have ( both times in a plaster cast.) It seemed to heal uneventfully never had continuing ankle problem. Started XC skiing during the Winter 6 or 7 years ago. Although I never had a bad fall when you do fall because your toe is attached and heal is free you do tend to torque your ankle some… it was two Winter’s ago it started bothering and never got better… it hurt to some degree all the time and sometimes worst just walking around or doing yard work then running. Tried cortisone shots, orthotics complete lay offs… nothing worked. Finally MRI and surgery. I’m hoping this does the trick… my activities have been limited a lot the last two years.
I haven’t had the surgery, but I just had an MRI that said I have a segmental interstitial tear of the peroneus brevis. My doctor pretty much told me it’ll just go away on it’s on in time, but after doing some internet research it seems as though I should be at least doing PT. Have you tried going to PT for your tear? How do you know if the tear is bad enough to consult with a surgeon? I’ve had my injury for only 2 months, but I am frustrated with the lack of progress and not being able to run or jump.
My type of injury would not heal without surgery and was difficult to diagnose even with the MRI. After two years of waiting to see if it would get better and trying all manner of non surgical intervention I finally decided to have the surgery. Physical therapy was not necessary because I didn’t have a weak ankle and the injury was not caused by some type of muscle imbalance or flexibility issue but trauma. The verdict is still out… I was hoping to be further along at this point. It still hurts but in a different way then it use to. Walking is almost pain free but I still don’t feel like I can run on it… I couldn’t even imagine playing racquetball or trail running.
I bike about 25 miles per day to help rehab total knee replacement. I started having pain on the outside of my ankle. X-ray was negative, but MRI showed split Brevis tendon. I reviewed options, got a second opinion and decided to repair it. Surgery was 6/10/13. Surgical wrap for a week, cast for a week, now in a boot, still non-weight bearing. I see the Dr tomorrow. My goal is to be back on my bike by 8/1. I kept off it, kept it elevated and didn’t have much pain at all. Looking forward to starting PT.
Had the surgery 12/24/2012. It was not successful pain is still there. There is a trigger point right bellow my ankle that if I touch is very painful and sends shooting nerve pain down to my little toe. Instep will get swollen and turn black and blue on occasion. I can run and bike some but if I do to much or to hard or to long it will be much worse the next day. Also feels laterally unstable like if you had sprained your ankle at it wasn’t all the way healed… sometimes cutting the grass is more difficult/painful then going for a run. I hope things work out better for you.
Hang in there!! So far I feel lucky compared to what I have seen from others with same injury. I have a high tolerance for pain, and have had 9 knee operations on the same knee, including total replacement. I have to stay active and keep riding. I have not been able to run since first knee replacement.
