I have a client who is a 35EE foot with a very high arch and am having some trouble sourcing a cycling shoe for her. We are looking for a lugged outsole and the best I can find so far is Lake Extra Wide in 39. She is also open to custom shoes.

This isn’t really a solution… I have 40 EE feet. Too small for men’s shoe sizes and too wide for Ladies. The only place I have been able to walk into a shoe store and find shoes that fit was Asia.
I’ve had Shimano and Specialized Wide shoes, which I just get 1-2 sizes too large to get the width. Even then sometimes I needed to cut vertical holes in the side, like gills, to let my foot expand wider.
… not really a solution.
There are perhaps some wider bont shoes which may also work.

We had a client do a pair of custom Bont for around $1200 in a similar size to your client’s. She was previously wearing Specialized Exos in 37 wide and so far the Bont have made a marked difference in her long rides.
