I don’t know of any differences between trip insurance companies, but my recommendation is, for the cost–get it. It not only covers you if you get sick or injured, but if you have to cancel and can’t get ticket refunds, etc. Consider my parents–twice on trips outside the US they have had to be hospitalized.

The Adventure Advocates plan is good worldwide and includes air evacuation/repatriation coverage as well as 24 hour accident medical and accidental death & dismemberment insurance (up to $25,000 per accident with a $100 per accident deductible), along with other non-insurance benefits. The program is currently endorsed by USA Cycling, IMBA and several large ski resorts.
i have travel insurance through American Express. I’ve never had to use it, but the policy seems pretty good. I’m not sure if they offer it to regular cardholders or only people on corporate/small business plans. Its pretty inexpensive and seems pretty comprehensive.
Just be sure to read the fine print. Most of these plans exclude participation in professional sports including training for such events. Many of them even exclude activities like “bicycling”. The www.adventureadvocates.com program was designed with athletes and cyclists in mind…
