So I’ve been dealing with some minor post tibial tendonitis. I was to the doctor for it last year they did not take any pictures but put me through a bunch of tests and told me it was not my post tib and just tried to pawn it off on plantar fasciitis. All has been well for quite a while until my mileage on my long runs has started to creep up and it’s happening around the same time that the symptoms started last year. I start hitting 9 and 10 miles plus and the symptoms return. I’m currently running in Saucony kinvara and also alternator in a pair of Hoka Clifton’s. Both are neutral shoes but the Clifton’s have more arch support. The cliftons are new to me just a couple months ago and I have not done my long runs in them but I do notice that I do not feel quite the same arch support in the kinvaras so my question is could it be late in the runs I need more arch support and I can borrow is just aren’t doing it for me? I’m a mid foot Stryker but I do supinate somewhat.
That’s a little odd because this injury often comes from someone with little arch support or pronates causing their arch to collapse. Other than rest you can try a foot doc and custom orthotics to keep training while it heals.
I shut down running immediately I’m 4 days without I started physical therapy immediately.I have noticed on my long runs that my hips get very fatigued so I’m wondering if the problem actually lies higher up the food chain

If you run your finger along the posterior edge of your (outer) ankle bone, as well as directly underneath it, you can localize where the posterior tib. tendon runs. Is it tender in this area at all? If not, where does it hurt? Because there’s a bit of difference in diagnosing someone with post. tib tendonitis vs PF.
As far as strengthening higher up the muscle chain, it would be good to focus on that regardless. It took me a while to realize the importance of a strong core/glutes but now I do. Not just the glute maximus but also the glute med. and min. These are VERY important to help control your gait/balance.

The pain moves around sometimes I’m guessing just referred pain but when things are hurting while running or doing other activities it’s right where the post tib passes under the ankle bone is where it’s the most sensitive sometimes I get pain down towards where I believe it inserts into the bone.usually don’t have pain running up the leg but on the occasion I do it almost feels like Achilles pain. Again I believe my case is very minor compared to what some people have but only because the second it flares up I shut everything down because I’ve seen what can happen when this goes awry.

To OP then by chance my thread on the first page on stopping using Orthotics after 15 years could be relevant to you - odd that this is two threads on the same (potential) diagnosis at the same time. I get the results of my ultrasound and x-ray this afternoon, but seemed that the ultrasound found inflammation on pretty much everything - PF, Post tib, achilles and not jsut on teh painful foot but the other one too…

This is a good post. I deal with tib post issues in clinic a lot and the focus on glute med in particular is key as a dysfunctional glute med leads to crossing over, which often causes overpronation and tib post issues.
In addition those shoes are doing you no favours. Kinvaras are generally the worst shoe for stability I have ever seen, with anything Hoka in a close second.
I’ve always run in a neutral shoe. When I’ve tried other things that have sled to shin splints. I wouldn’t say I have extremely high arches but I have reasonably high arches and tend to run on the outside of my foot… I always get the terms mixed up but is that supinate?
