Nerve impingement / Ummm, I'm doing something wrong... (Newb / Male anatomy / Not-for-the-ladies question)

EDIT: Just thought I’d give an update since this post is still going. Last night I still felt a little off. I wasn’t in pain but I had discomfort. Do you know what ghost limb is? I felt like I had a ghost testicle! Almost as if my ascended testicle was still riding up in the canal. Brutal. I went in to the emerg to get things checked out because I realized I could either: 1. spend 4 hours waiting for a doc, or 2. lie awake all night envisioning having a testicle lopped off. Anyway, I was sitting there and slowly figured out what the problem was. I knew it wasn’t a twist because it would have been painful and everything looked OK. I was pretty sure that I had pinched some nerves back behind, which caused a cremaster muscle to hightail the testicle out of there. The discomfort I was feeling was residual effects from impinging the nerves. I also thought that maybe I had cut-off a bit of the venous flow, leading to the slight swelling. The doc agreed with me, everything was intact and as it should be, most likely some nerve impinging. Had to have the boys fondled but it turned into a full testicular exam which I have never had before. So not a waste of time at all. And she put me at ease. (I hope you guys check yourselves out. Be diligent.) Anyway, moving forward I think I’m going to fiddle with my saddle position a little bit and then consider swapping it for a different one if that doesn’t help. Summary: if you are starting out and reading this, don’t ride through pain that just doesn’t seem right.

The following question is odd, but I’m really not trolling. I wear decent bib shorts when I ride - they fit properly, feel decent and are of good quality. However, about 10 minutes into any ride I always start to feel uncomfortable. I don’t really know if I don’t have them placed just perfectly or what but all I know is it isn’t fun. I try to “adjust” myself but it never really lasts. There’s just too much pressure…

Today I went for a 19 mi / 30 km ride and was quite uncomfortable. Whatever, got through it. When I got home though I was standing in my kitchen and pulled out the front of my shorts for a moment to adjust again. Looking down, I saw a large, very out of place lump to the left of the base of my penis. Obviously I felt this anomaly It was one of my testicles, which subsequently dropped into a more familiar arrangement. Surprise/horror face. Basically, one of my testicles was fully retracted all the way up to the front of my groin! This has never happened to me in any other scenario and just can’t be good. If this is a common occurrence then you are all crazy… Now I realize that the pressure and discomfort I had been feeling in the same place on most rides must have been this same situation! This wasn’t one of those I-just-jumped-into-a-cold-lake ordeal. The unpleasant fact is one of my testicles was in a scary place as a result of the way I ride or the way I’m wearing my shorts. No way am I putting up with retracted testicles.

This is the part where you laugh; bestow on me some godly, exclusive, little-known bike short advice; cry for me; point me toward a link that shows you exactly where your junk should be (not sure I want to see such a link); or otherwise give me help so that I can ride in comfort.

Additional info: I’m 21, my bike is properly fitted (although I want to move the saddle forward), my shorts fit and are of good quality and my endowment may be considered a bit on the larger side (absolutely hate stating such a point but perhaps it is relevant or helpful…).

P.s. Just realized I have more detail than necessary. I apologize. But mayhaps it will be useful. HELP ME!

I realize this is only for the Adamo saddle, but it’s a picture that shows where stuff should be and you asked for one.

If you are even a little uncomfortable you need to try a different saddle. I have a Cobb right now and its about 90 percent there but not perfect so I’m switching to a Selle SMP which I used to use and liked. There is no reason to be in pain on your bike.

That picture has always baffled me. Sure, if you’re riding naked that’s how “you may be hangin’”, but put some good fitting shorts on and things get a little different!

A few responses for the posts above:

  1. I don’t have any money for a new saddle and my LBS isn’t even local, it’s 40 minutes away. Picking up a new one would be too costly. Also, I don’t have this type of trouble when I ride to somewhere in town wearing my paper-thin running shorts (although the time is much shorter).

  2. Starting out, I spend most of my time on the hoods right now, go into the drops rarely when windy (road bike). After a time, the only way to be comfortable is to stand and adjust :confused:

  3. I agree with atomic in that, with bike shorts on, I don’t think there is really the opportunity for everything to sit like that on the saddle… Still a helpful picture though.

  4. Saddle is parallel with road.

EDIT: I might try sliding my seat forward a bit on the rails. I found during the winter when I spinned that I found it more comfortable to be a little more forward.

save up money and get a new saddle, I know you said you cant afford it right now but it is one of the most important pieces on your bike. Sell something, eat cheap, what ever you have to do. Just my two cents but spend your money first on saddle, good cycling shoes, and good kit. Comfort makes all the difference in the world when you go long suffering differences.

From a medical perspective- make sure you are not experiencing testicular torsion (google it)…usually painful, testicle “twists” and elevates…this can be a big deal. Hopefully something else, but thought I should give a word of caution when such an area of high importance is involved. Good luck.

Try not pulling your shorts up so high. Let the boys dangle prior to getting on the saddle. tipping the nose of the saddle down slightly probably will help too. I experienced this once or twice. I didn’t change anything in particular and it stopped… I think it was the shorts being pulled top to high and tight. Good luck.

Question: are you wearing underwear with your bibs? Because you should not.

Are you tall, or at least all relative to your waist size? I ask because the shoulder straps may be too short for you and are the culprit for cinching your bits.

Lastly, I try to make sure the legs of my bibs are pulled high enough such that the material for the leg comes all the way to the crease of the groin, allowing for a “pocket,” if you will, in the lycra for my junk to hang in.

It may also just be the bibs. Even if they are good quality ones that doesn’t mean they are good comfy ones. I had to wear speedo and tyr for worlds and they were just miserable pads in them.

Your junk needs to be pulled up and out of the way.

Don’t sit on your bag, your marbles, or your gun.

If your junk keeps sliding down then your shorts are likely not doing their job in that region.

As suggested, a different saddle might fit also you better. Like shoes, everybody has slightly different needs.

I was ready to give up cycling all together because of retractile testicle last year. I discovered 2 game changers that have allowed me to stay in the game.

  1. Save up & get a Dash saddle!!!
  2. Take your shorts/bibs into a tailor & have any padding cut off & removed that goes up under ur nuts. Meaning… the pad stoops at the taint/sack junction. You don’t want any upward pressure.

It’s funny… I have race pics where I’m in the process of adjusting. I had to un-clip my right shoe, kick my leg out to the side & push down with my fingers to pop my ball back down into the scrotum. And that was every 10-15 minutes!! Lol. Hurt like hell though!

You say your shorts are good, your fit is good, etc. If all of the above are true, it probably wouldn’t be happening! Tell us more about your setup.

As with most things there is a spectrum of presentation from the dramatic/obvious to subtle…I have seen a few patients who did not realize this was going on. One key is the pain, when blood flow to the testicle is obstructed (from the twisting) it causes pain. If the testicle is elevated, not painful, and positioned correctly (same as the other side) it is very unlikely to be torsion. I think his sounds unlikely to be so, but again I would not want to lose a testicle. Seems likely that some junk positioning, bib, saddle adjustments will resolve his issue.

I actually don’t think its your saddle. Everyone is so quick on here to say saddle, but I just don’t find that to be the case.

A 19 mile outdoor ride should not be causing whatever anatomy problem you described. I highly doubt any saddle would even cause such a problem either.

I would suspect its whatever clothing you are wearing, you are probably restricting certain areas that simply shouldn’t be restricted. I don’t wear bibs, so I am not sure how tight those are, but what about just regular bike shorts with large padding in the sensitive area?
