i have pretty wide feet. not wide enough that i need EEE or whatever, but pretty wide, especially by narrow bike shoe standards.
sidi makes a wider model called a mega that is pretty wide. for tri shoes i find the bontrager hilos to be nice & wide. soft mesh too so you get lots of room.
i think i recall others saying that the northwaves are wide.
Check out BONT. They are the widest I’ve seen and can be custom fit.

I have a similar foot , got Lakes, but no carbon sole.
Bonty Hilo. They helped me.
I have a wide forefoot. Got the Shimano RH220. Available in Wide, and is heat moldable. Carbon sole. A great fit.
Some options:
Sidi- Mega
Specialized- come in wide sizes
Northwave- probably a bit wider than the Sidi mega, no idea on the quality though.
As much as I love tight box, I do recognize the need for a wide box to put my trick ski feet into. I have used the Sidi Mega’s with great success and comfort. Such is the dilemma, tight box and loose box. We CAN have the best of both worlds.
My feet are wide enough to give me fit problems with all sorts of footwear. I’ve had very good luck with Lake shoes and the Sidi Megas.
Like everyone else has said…Specialized and Shimano= great, non-mega Sidi and Vittoria= not good.
I had a pair of mtb Northwave shoes a while back, I remember liking them…never tried their road stuff.

Don’t know about other Shimano shoes, but their R132’s run narrow and long. Bontrager’s road and tri shoes are all wide in the forefoot. I have the Hilo’s and RXL’s.
I have the R160’s…same as the r220 just without the buckle I believe. Coming from a Specialized 48, the R160 in a 47 fit perfectly- much wider than the Vittoria I was hoping to buy at the time.

I tried Specialized, Shimano, Vittoria , Sidi (wide sizes worked but could only find road shoes), and Bont (was a little skeptical).
The Louis Garneau has a wider toe box without having a wider heal so it fits great.
I wear the Sidi megas for a road shoe and like em, but they unfortunately don’t make the tri shoe in a mega.
