Major foot pain in bike shoes - how should I adjust my cleats?

Finished my first 70.3 a few weeks ago but was PLAGUED by pain in my feet from my bike shoes. Okay, I’d only ridden in them a few times prior to race day (yes, dumb I know) but they’d always felt fine at sub 30 mile distances. After the bike I could barely run due to the foot pain.

So here’s my question - anyone know how I should adjust my cleats to fix pain (and swelling) on the outside edge of each foot, from the ball (but on the outside edge) back about 3 inches? I’m wearing a pair of SIDI Zephyr shoes and use Speedplay clips. I know SIDI run narrow but I have a narrow foot and the basic fit seems fine so I’m thinking I need to tweak my cleats.

Any suggestions?

rather than have a bunch of people who haven’t seen you on the bike offer advice, take it in and get fit by a professional who knows what they are doing. Someone on here telling you to adjust them or cant them could do waaaay more harm than good, like a pandora’s box situation…knee pain, etc. Have someone take a look.
