Just thought I’d post some of my pain and symptoms to see if any ST’ers may have had the same issues and/or some advice at to what you may have done to alleviate pain.
After about 4-5 miles, my right knee feels like it gets too loose. Some minor pain at impact, but the majority of the pain comes in the part of my stride when I am lifting my right leg and motioning forward before landing.
I did have ACL repair surgery back in 2000. All has been good since then. I’ve wakeboarded, skied, played basketball/volleyball, all without problems. I have been increasing my running steadily for the past 3-4 years. I completed a HIM back in July with relatively little knee pain. However, I have been increasing mileage as I’m training for a marathon in October. After about 12 miles, the pain is enough to make me stop running. If I stretch and massage it a bit, I can continue for a short while longer.
I haven’t experienced any swelling. The pain seems to subside a few hours after running.
I’m hoping someone may have some diagnosis/advice for me.

Over use injury. Sounds like the way my knee was feeling. Same with me, no swelling and the pain went away.
I got an MRI. I backed off on the running a bit from 50-60/week to 10-15, got a pair of flatter shoes - Saucony Kinvara - the heavy stability shoes seemed to make it worse, and used one of those patella brace/straps (http://www.trivillage.com/prtipr0100.html) to stabilize the cap. Iced and elevated. Gradually I’m building up to 6 miles a couple days a week and can now run without the strap.
Try the knee brace strap and cut back the miles. It may still be a bit painful for a couple runs, but after two weeks of taking it easy the pain may go away like it did for me.
Where is the pain? I know the knee … is it on the medial, laterial, front, back …etc.
Lets start there…
Pain is in the back of the knee. Almost feels like after a few miles, the ACL gets stretched out and allows the knee to pull apart.

I solve my level 2 Chondromalaci with Glucosamine/Chondroitin/MSM. Still taking it just in case.

Okay, so after about a week of trying various exercises, braces, stretches, I’m finding little (if any) relief.
I do feel, now, that the pain is more toward the outer side (medial/lateral??) of the knee toward the front. There is a tendon that pops back and forth as I bend and straighten the knee. It doesn’t make an audible “pop”, but you can see it under the skin. It doesn’t hurt to just make it pop, but I’m wondering if the constant movement builds up the pain and may be why it only hurts after a few miles of running?
I’m thinking I may have to get an MRI to see what’s going on, but please let me know if you (or anyone) might have some further suggestions.
