I’ve got over 43,000 miles on an old dinosaur spin bike, that I set up to my tri bike coordinates, aero bars and all. I have never lubed the chain. I’ve never even even taken the chain guard off. The thing still runs as quiet and smooth as it did, the day I got it. I’m assuming the chain was probably greased at the factory.
I am terrible. I ride 4x/week ~ 6 hours total. I usually wax lube mine when I remember that I cannot remember the last time I did it. Probably every 6 weeks. Dino lube could probably go forever.

Same amount you would as if you were outside riding, friction happens inside and outside.
By contrast-I change my chain to a freshly waxed version every 6-8 weeks depending on bike volume.
Not very often. Maybe once or twice a year (I typically ride 3-4 hrs/week on the trainer)? It definitely doesn’t get (or need, I think) as much care and feeding as my outdoor bikes.
Probably every 1-2 weeks, but if anything that’s less due to the lube itself and more because the fairly magnetic attraction to cat hair seems to require me to clean it fairly frequently.
My experience has been that lube holds up way longer indoors though- I would think twice a day would be fairly unnecessary unless you happen to really like lubing chains.
I do mine the day before I do an FTP test which typically happens about every 4 weeks.
I started waxing my chain, because kids would touch a greased chain and make a mess
I re-wax when it starts to get noisy / not smooth, which is every 3-4 weeks or so
