Hip Labral Cyst and Debridement

Has anyone ever had either this condition and if so did you have the treatment? It is an arthroscopic procedure. I have a cyst on the labrum of my right hip that has caused me awful pain while running, particularly off the bike. It also can cause debilitating pain if I move the wrong way. I have a consult with a surgeon in a couple of weeks to see if he thinks he can help it or if I am in a situation that requires “activity modification” per my referring ortho doc. If that is the case I am out of the tri game. Since I am pretty slow it is not a big loss to the sport but I really do enjoy the events and the discipline it requires. I am now just biking and swimming on a regular schedule and throw in a run once a week or so.

If anyone has ever experienced this and is willing to share their thoughts I would appreciate it.

I didn’t have a Cyst but I did have a Labral Tear in my right hip as a result of Femoroacetabular Impingement (FAI) and had surgery to repair it about 3 years ago. I am a big guy and I thought my days of running were over for sure. In fact when the doc gave my wife a summary of the surgery while I came out of the operating room he said straight up that I was not to run, especially at distance.

Last month I did Ironman Mont Tremblant in 12:06. Not blazing and my run was slow but I was able to do it.

A couple things worth sharing:

-before surgery I was in the same type of pain as you. I had a hard time walking at times.
-I had a typical MRI and a Magnetic Resonance Arthrography (MRA) - - - You want to get the MRA! Its the best way that the docs can find the problem, but I found that it was quite difficult to actually get it because its expensive and not that typical. Oh and they insert a needle full of dye directly into your hip! Scared of needles?
-If you are really smart you will be sure to have married a physical therapist to ensure that you get back on the road!
-I still have some pain when running, but I have learned to manage it better and if things are getting bad I take time off or cross-train somehow. I know that my running and triathlon days are numbers so I am enjoying them while I have them and trying to not be dumb!

Good Luck! Let us know what happens next.

Cysts in the hip joint are usually associated with a labral tear. Hope that yours is not. Very little good research evidence on prognosis after arthroscopic surgery.

I want to give a quick response to the post by bigendurance. I have recently had FAI surgery with labral tear repair as well - and my experience has been very different. My surgeon was very positive about my return to running, only made me take 4 months off and back to 90% in a year. Your prognosis will probably be different based on age (I’m early 20s) and length of time with the injury - I was told that the worst case is when guys put off seeing the doctor for years and end up needing microfracture or a replacement.

I just wanted to make sure you don’t get an awful impression of recovery time. Send me a PM if you’re in the Bay Area or Seattle, I could point you toward some great surgeons in both places.
