I thought, when I clipped in for the first time with the new shoes, that my feet felt like they are slightly angled on the pedals. (Tilted a bit towards the outside.) Which felt a bit weird at first.
(And I replaced the std inner soles with the medium arch support version right away, before I even used the shoes.)

Specialized shoes have some varus wedging built in. Not sure how adjustable/removable it is, but worst case you could use a cleat shim to undo this and re-level the shoe.

I am 99 percent sure his problem is Varus wedge that is built into the shoe. I never felt quite right in my specialized trivents and have gone to Shimano

Hey knowledgeable folks!
I could use some help identifying (and hopefully rectifying) the cause of knee pain when cycling.
I never had any issues with knee pain, once I got my old Mavic cycling shoes set up for me. I’m riding 3 pairs (all Mavic, all the same size, all setup identically) with no issues.
I recently got a pair of S-Works cycling shoes, set them up the same way I did with the Mavics. (Cleats are in the exact same spot relative to my foot, no shims or anything special.)
Now - if I ride with the new shoes (that are otherwise excellent and comfortable BTW), my left knee starts to act up after about 20 Miles, followed shortly after by my right knee.
I feel pain on both front knees, towards the inside, just above the knee cap. Sadly that also effects the run off the bike.
Any tips, hints, advice would be greatly appreciated.

Yup, after a bit of reading into these shoes, I think that’s the issue.
Specialized actually offers internal wedges to even out the built in varus “wedge†and transform the shoe to “neutralâ€.
I ordered a set and hope that will solve the issue.
