Has anybody had trouble with an Easton EA70 stem coming loose and/or not clamping a carbon steer tube tight enough?

I’m using an easton EA70 stem with a carbon steer tube Reynolds fork on my road bike (w a Campy Record integrated headset). I’ve been having problems with the headset coming loose. I’ve checked everything, and it appears fine. I can adjust it properly, ride a week or so, and it loosens slightly so that there is a small amount of play.

Today I noticed that the Reynolds expander bolt assembly (the part that wedges into the steer tube and the headset top cap threads into) had risen a few millimeters inside the steer tube. It appears as if the stem doesn’t quite tighten enough, and over a few hundred miles slowly loosens, pulling the expander bolt assembly up with it.

Has anyone experienced a similar thing with this stem?

thanks. my suspicions confirmed. i’ll replace it today. any suggestions? I want something that works … put it on, tighten it down, and forget about it.
