this is entirely new to me… same Look pedals and Carnac carbon shoes I’ve been riding in for decades, suddenly have intense pain in the ball of foot/toes and sensation of pressure, after about an hour of riding. It’s happening in both feet, worse on R. Cleats are already set as far back as the shoe will allow.
The pain is sufficient that I have to stop and unclip and breathe for a couple of minutes.
It’s not the neuromas which I’ve had for even longer than the shoes. The pain going up into the toes makes me think it is some other kind of nerve impingement though. Running injuries I know about, cycling injuries are a new thing to me…
I think of changing the insole, switching to a backup pair of even older shoes, ?
any/all ideas welcomed…

one idea might be to add a bit of a bump under the insole under the metatarsals to spread them out. I did this in my insole and found it works. I piece of old bar tape seems to work for me. Cut about 1 inch and place it running front to back.
I recently had some pain in my first metatarsal, right on the ball. It felt like I was pedaling on a rock. I think it was turf toe for me, but sounds a bit different than your pain.
Anyways I’m pretty sure mine were caused by doing calf raises and walking barefoot on hardwood floors all the time. I changed those two things and the pain went away cycling. So maybe just double check there isn’t something outside of cycling that could be causing it.
I have a torn plantar plate, which makes my toes feel like I’ve got a piece of string tied around mt 4th toe super tight. I got insoles with a metatarsal pad and it helps a ton.
age 64, everything is changing… it’s like being a teenager again, every year the body and its responses are different… but in a bad way
have metatarsal pads in all my running and walking shoes for the neuromas, do not have them in the cycling shoes as it hasn’t previously been a problem. But that’s a good idea, will try…
pain in R foot is there sometimes in the morning, immediately upon getting out of bed. I remembered this only after getting the pains on the bike, which are in both feet oddly. But there may be something new happening in the feet I guess.
yes, feet and ankles swell as the day goes on, usually exercise will reduce the swelling - I sit too much, earning a living behind a keyboard and 3 monitors…
