Having issues with my right foot. After running 30mins or more I have a burning feeling that extends throughout the medial portion of the top my foot that extends all the way through my big toe. Hardly trained for my last Half Iron run due to discomfort which at the time I thought was a possible stress fracture. Went and saw a D.O. who informed me he thought it was simply nerve damage and that I could still race. He “built” up a pair of green Superfeet with additional mole skin-type product under the arch and under the pad (on the under-side of the orthotic). I raced (with the orthotics) and felt ok through the first 6 miles and was in absolute misery through the last 2 miles of the run. Foot felt fine the next day.
That was Aug 1st. Have run 1 time for about 45 mins before I had to stop and walk. I bought new shoes yesterday and ran for 15 minutes last night (without the orthotic). Felt ok but thought I could feel the beginning of a similar “burning sensation” through my large toe. Last night and today my big toe is feeling kind of stiff or arthritic.
I signed up for IMWisc 2010 and am trying to begin a run frequency program. I have a very large arch and am a supinator, am 6’4" 195lbs.
Please let me know if there’s any other information you need that might help a diagnosis/suggestion. I am grateful for knowledge you might be able to pass on so thank you in advance.
Was a simple X-ray taken? No great toe joint pain or true stiffness/limitations of motion (not just the feeling of such)?
Deep peroneal nerve entrapment can manifest like your description (as can saphenous nerve, depending upon what side of the great toe is involved). If you can further localize what side of the great toe seems involved with the midfoot burning, it may be helpful.
Lastly, if the pain is reproducible (i.e. go for a run and it WILL happen for sure), a selective nerve block can be very helpful in isolating the problem.

They took three x-rays, all came up clear of a possible stress fracture. The pain seems to be between the joints on the big toe, but possibly more lateral (for the distal joint). No limit on range of motion.
The pain from the dorsal portion of my foot seems to lie in between (what I would describe as) the first two sets of meta-tarsels (from medial to lateral) from the top of my arch through my great toe.
Are there certain tests to indicate which possible nerve? Possible causes? Dangers of a Nerve blockade?
Ran for 15 minutes today at work but on a different tread mill, no effects.

With your foot hurting as you describe, I definitely would stop running and go see a doctor. I would suggest listening to your body and avoid worse damage.
