so, doc gave me and prescribed an NSAID I’m not really wanting to take, (Severin, nimesulide) and I’m wondering which NSAID is the best for inflammation of trochanteric bursitis - keterolac, diclofenac or plain old ibuprofen or something else. I need to get this fixed this week so I can get back to training. I’ve been doing nothing but swimming for 8 days now.
Might want to ask your (non US I assume) physician why he is choosing that particular NSAID (it was never approved for use in the US) since it has some liver toxicity issues.
Having said that, there really is no significant difference between ibuprofen and diclofenac at similar dosing. Ketorolac is not used often in the US orally (often though as a single pre/post-op IM injection though) since pretty rough on the GI system.
Personally, I’d just have that sucker injected (if you can find a specialist skilled in this). Way more effective and less side effects in most cases if you must have it treated.
yeah, not in the US. He had samples and said not to worry about what was on the internet. he said the bursal inflammation has gone down 70% in the last week and i guess i really should have gotten the injection a week ago when he offered it. Doh! diclofenac and keterolac are all very common, oral OTC meds here. i guess I’ll stick with ibuprofen then. do you think I should wait out this week and start training again next week (his suggestion) or should I get it injected ASAP (he does it via ultrasound)? 6 weeks to IM now…
If you are “70%” better, then probably in the clear. The problem is timing, you should be in your sweet spot in training right now then if a planned IM in 6 weeks. Likely an overtraining issue then I assume, so careful resuming.
There is some data that diclofenac has higher joint penetration.
I always go with naproxen sodium as it is dosed twice daily and works well
Ask about the Flector (diclofenac) patch, which can be applied topically for up to 12 hours. It has proved helpful for some persistent PF.
Pool run for a few weeks, you will not lose any fitness. Those drugs rarely work without side effects. Read this and do as much as you can stand.

TX so much!
Kathy I was given diclofenac for shoulder pain/swelling after having surgery for a labrum in my right shoulder. It did nothing for my shoulder but at the same time I had PF so bad as well and it was GONE after taking the diclofenac for three days. It does come back if I stop the med but it wasn’t near as bad as without it. You are only masking the pain but it does reduce the inflammation very effectively. My doc said there is no long term affect but still I don’t want to be on anything long term so I am trying to wean off and use only when it really really hurts. I would give the diclofenac a try.
