Diagnosing Dull Hip Pain

Hi All - First and foremost, there are a lot of great threads on hip pain on slowtwitch. The threads actually lead me to posting my issue here because it caused me to second guess my issue.

My dull hip pain feels like it’s coming from where my right leg meets my hip and/or around the sartorius muscle. It started 2.5 weeks or go … my guess is from running hills and/or skiing … neither of which I was doing prior to 2.5 weeks ago. I just wrapped up a rest week with stretching/icing with only very minor improvement.

I have no pain when I sleep, stand still, stretch (standing lunge, kneeling flexor stretch, butterfly, bridge, etc.) swim, cycle, sit, etc.

I the dull pain mostly when I walk, go up stairs, or run. For example, when I lift my right leg up to the next stair there’s no pain, but as soon as I start pushing down on that leg to get me up to the next stair … I can feel it. I also feel it when I walk, but only when my right leg makes contact with the ground and supports my body.

Running in place doesn’t seem to bother me, but once I start going forward it’s very noticeable and I basically prevents me from running.

I would suggest seeing a hip orthopedic specialist and get an MRI. You won’t really know for sure until you get an MRI. But if I were to guess, my bet is on a labrum tear.

In the meantime, work on strengthening your glutes.

I developed a dull pain in my hip whilst marathon training a few years ago.
It was strange in that it eased off once I was warmed up until it didn’t.

Anyway, turned out to be a stress-fractured femoral neck and I avoided a pin and surgery by the skin of my teeth.
My advice would be to get yourself an MRI if only to rule out anything serious.

Labrum. I did the same thing to myself years back during a hilly half, tore both Labrums. After many hours of wasted PT time, I went in for the MRI. I should have done first. I ended up getting a few injections and 12 years later I have not had any issues.

I second this. My wife is going in for surgery for this 1 month from tomorrow. Very similar symptoms. Good news: she had the same surgery back in 2018 on the other hip and it has been a very positive low-maintenance experience since for the surgically repaired hip.

Worth seeing a professional about… I saw one years ago after I was sure it was something serious going on. I have a few uncles with fake hips and wondered if it was my turn. He told me I was fine, just needed to stretch more and look for some Yoga for runners videos… I found this one.

I just focus one doing the stretches and honestly not consistently… only when my hips start bugging me again. It does work for me though and my hip discomfort.

Thanks everyone for the helpful input. Took your advice and saw an Ortho w/ focus in Sports Medicine. Got an Xray, which was negative. Got an MRI and read out was today.

This was it - right hip labral tear. I’m glad I went in and saw him ASAP. I think it was either from sprint intervals on incline (treadmill) or downhill skiing. He did clear me to run so long as the pain wasn’t bad. I’ll get a cortisone shot Tuesday and then will do some PT to identify and weak areas of my hips. Hoping my outcome is like yours!

Curious do you still run hills today or avoid them? IMLP has some elevation, but nothing crazy. My IM plan encourages hill sprints, maybe I stick to sprints w/out a hill? Will ask my Ortho the same question.

I recommend you take some time and ease back after your injection. Lake Placid is a long way out, I would keep your runs flat and easy until all the inflammation is gone.
