Debiltating hip pain - on more information might be more ass then hip

I’ve noticed hip pain (both sides) for about a year now and thought it was related to stretching (or lack thereof) and really only noticed it on super long runs (over 21km) or on bike rides (of any distance) but it didn’t hinder me. went for a bike ride tonight, came home and getting off the bike hip was achy and I was limping. now hours later and i’ve iced and ibuprofen (400mg) and it is SCREAMING. I cannot adduct without searing pain but abduction is possible. hurts on the outside of the hip and radiates to the knee (when standing). hurts passively and actively. hurts at rest. if I lock my knee and keep my leg back, behind my pelvis, i can “walk”.

obviously going to the doctor tomorrow but no trauma - WTH could this be? Ibuprofen isn’t touching the pain, just took 50mg of tramadol. Have a vial of dexamethasone I can have husband inject and also prednisone if that would help.

thanks for the ideas guys. am able to shuffle this morning and woke up last night when the tramadol wore off but I was very happy that I slept at all. googling all the above as Dr Google last night kept telling me arthritis or fracture. have a race on Sunday and an IM end November. If gluteal tendon tear, any idea if the IM is going to happen?

also post google, it looks like no reason to rush off to the doctor - husband will be at immigration all morning so wouldn’t be able to go until the afternoon. any reason I should make the effort (will involve getting someone to take me to the hospital/doctor) to go today?

none. found a PT clinic about 85 km away.

Do you have any “knots” or steel cables in your quads or hamstrings?

Let massage be a starting point. Go to the fastest runner/triathlete in your area and ask them who they use for massage. Get to that person immediately. You’re going to have to see that person 2-3 times a week. It will hurt, but you’ll probably experience relief. Make sure to go back a few days later. Since its been ongoing for a while, it will take a while to work it out. Commit to at least 3 weeks of massage work. Anything running down your quad/it band can come from the back, TFL, ITB, glutes, glute attachments, etc. you need to have your entire “chain” worked on and loosened immediately.

You’ll know if it’s working. In a month, if it’s 0% better post again and I can suggest the “next step.”

Some great news, you’re about to get faster. No massage equals really tight muscles which equals muscles which don’t fire properly. I’ve been through it, start here.

MD’s are great, but most can’t FIX overuse injuries.

I kind of agree with this. But I will add that you STILL should see a doc to make sure it’s not a full tear or other structural issue.

OP: Be prepared to not run at all for a bit. I did the doctor dance and included massage, chiro, and acupuncture. Massage and chiro helped. Chiro adjust me so me pelvis wasn’t tilted anymore. Massage to loosen the quads and hamstrings.

What I have been doing for the same problem along with massage is to still swim and cycle. If you are going crazy about the running bit, like I have, go to a gym with a treadmill. Start off just above a walk pace and slowly increase pace. I start at 9:30 pace and drop 15 seconds every quarter mile. Sometimes I need to get off the treadmill in the first 2 minutes as I can feel a twinge coming. I foam roll my hip, quads, and glute and it usually loosens me up to run again. Listen to your body and respond by stopping immediately. This is why I have used a treadmill in a gym with a foam roller. I can stop, foam roll, start again. Also, the treadmill is flat with no crowning or potholes that can through off your gait. I was trying this by running outside but would get 1 mile away from home and have to limp home. I also found doing some leg and lower back strength exercises are helping.

After 2 weeks I’m up to running 20 minutes per day. Frustrating as hell as I was running 60+ miles per week up until 3 weeks ago.

I hope the free speed with loose muscles is true. This season was horrible for me and I never got past nagging hip issues all season and my run and cycling really suffered.

Always good to see a doc to count out catastrophic injuries. My gut tell me, since its bilateral, not a dual tear. Couple that with never having any body work, and you’ve got yourself a mess.

I’ve been there done the dance for 3 years and didn’t get it all fixed until I paid for platelet therapy to fix the back and glutes where they attach to the quads. The glutes could simply be pulling her quads down and back which also tightens the IT Band creating a situation where someone can’t even walk. Been there, brutal.

Not on the hip. That sound painful as hell. The point for me is near the pelvis crease all the way to the outer hip. Lots of other bones/tendons/bits to get in the way of graston.

I had something similar, maybe it’s what you’re experiencing. For me, it was a referred pain from a pinched nerve in my spine. Dad has it too. What works is a stretch that I do a very hour or so for about 20 seconds. What you is find a table, turn your hands so your palms are facing away from you, grasp the table, lock your arms, and lean over the table, stretching your back. It feels like you’re trying to separate your torso from your bottom half. The action itself looks like you’re trying to push the table into the ground.

Hope that’s clear, maybe give it a try, it saved me.

i’m icing more my ass than to the side of anything. i’ve had pain on both sides over the past few months but I never really paid much attention to it - it was very mild and seemed to abate with stretching. i’m feeling myself up and don’t think my legs are super tight (quads, hammies) but after sitting I’m VERY stiff and/or the ice makes it worse? Still waiting for physio to contact.

no - symptoms only on teh left hip (that is my short leg and usually my very loose side when I stretch). No low back pain at all. took tramadol only so I could get some sleep. doing tests now. thx! SLR produced a very, very mild ache (maybe) very low down but no real pain. Getting down on the floor to do the damn test HURT though. :slight_smile:

If stretching is helping, go get deep tissue massage. It’ll take a while to loosen fully. The longer it’s put off, the worse it gets.

I just had the same symptoms start back in August training for the NYCM. That is kaput now as I couldn’t run because the hip hurt so badly! Had an x-ray which was perfectly normal, but then the ortho guy suggested having and MRA, not MRI. MRA is where they shoot dye into the hip to highlight any tears. YUP turns out I have a labrum tear…I am @#$#@!!! Dr. says I need orthoscopic surgery to either debride or remove the tear. I’m not so convinced I am going to go through with this.

I had a steroid injection which after 2 days has now brought total relief, ran twice now for 2 minutes each with no pain…I know…woot.sigh. But I’m looking into a hip specialist guy here in INdy and another up in Chicago on the 21st, I wouldn’t want it to be further damaged by being obstinate about surgery. I’m exploring EVERYTHING before I let anyone do surgery on me :slight_smile:
