Chronic Patella/IT Problems

Hey all I’m posting here seeking advice on knee discomfort related to the IT band and patella. About ten months ago building up for base training I started noticing a popping senstation coming from the top anterior part of my right knee about where the IT band joins with the lower part of the leg. Intense swelling made it hard to walk, so I dialed my training back and took some time off. 3 months after the orignal onset of the sypmtpons I finally went to a orthopedic surgeon who diagnosed with me chondromalacia patellae. I was perscirbed antinflamatory medication while I countineud to rest. Unfourtnatley my symptoms did not subside. I went to a physical therapist and worked with her for two months. She belived my problems were assicated with weak glutes and tight hamstrings, which are common problems in runners. So we worked on excercises strengthing those areas including my hips. After two months I lost confidnece in her ability to solve my problems based on her attiude towards my personal situation and the lack of knowledge related to treating knee problems, so I left her practice. At the moment I am currently looking for a new physical therapist and am constantly doing stretches and excercises realted to knee strengthing.

What I think I need now is someone who speicalizes in gait analysis to look at my stride to correct possible biomechanical issues. I think a maltracking patella might be the primary issue here.

Just to recap: The popping on the anteirior top of right my knee feelings like something is moving out of place every time I fully flex my leg. I feel the tendon move back into place when I close the angle on my leg again. I can physically push this tendon with my fingers and can feel it moving over pieces of bone.

Have any of you had issues similar to this? If so what would you reccomend in the next steps of treatment? Any comments or advice would be appreciated.

chondromalacia is a bitch. There’s no easy answer. Rest and PT is all you can do. The therapist you didn’t like was on the right track, but find another one if she wasn’t working for you. But, don’t expect instant gratification when it comes to chondromalacia.
