I’m in a 48 Carnac TRS6 that is bit too tight on one foot causing Mortons Nueroma symptoms, not good. I wore out a pair of 48 Comets recently that fit great, perhaps its just the TRS6 design. Got a bike shoe recommendation for a 48(+) foot?
I find the SIDI T-1 very comfortable. A size 48 fits my wide feet (13EE).

funny how everyone has different comments about different shoes - I wear a 4E 9.5 in asics, and found the sidi genius 4 mega’s not that fantastic. My shimano tr-01’s are perfect; although I am wearing them without socks which really sucks as it got cooler…
I was out at the running store last night trying on shoes, about 15 pair, everything from a 12D-15D from a 12.5 EE – 13.5 4E…quite a range on how the respective shoes companies size their shoes. Wonder if its the same with bike shoes.
I agree with one post / report ,that Sidi bike shoes run about 1/2 size bigger that the Euro to US size charts shows . I have had great luck at sales or online, with shoe sale of 60-80% off on large sizes. I just found some 48 Sidi Mt. bike shoes at $30.
If you need a tri shoe bigger than a 48, you’re out of luck. The Carnac’s seem to run the widest of the shoes on the market right now; maybe you could try a different (newer) model. Another option is the Sidi Dynamic which comes in sizes up to at least 50 and although it’s a road shoe, it has straps similar to a tri shoe (open out).

Try Rocket 7’s they make custom shoes fairly resonable too all things considered.I’ve had to use them for a few customers.

I have had a lot of luck with the sidi genius version 3 and 4. I wear a size 15 (US) running shoe, and the fit of the size 50 sidi is great with an orthosis inside. Without the insert, I might need to go down to a 48. They are not, however, “tri-shoes”.
Random thought for today: I just learned that orthosis is proper term for what we all call an orthotic. You put an orthosis in your shoe. Orthotic is the adjective. In a sentance, we could say that “Philbert is a gigantic goon with canal-boat sized feet and bad arches/pronation who required orthoses made in an orthotic lab so he is not hobbled by achilles pain.”

Specialized makes a tri shoe to 14. I’m a 15 in running shoes but can usually get away with a slightly tighter cycling shoe. Nike 14s have always worked for me as well. Their toe box is well shaped to fit my crazy 2nd Mortons toe.
