I have been struggling with my right foot for a couple years now, and can’t quite get a grip on it. I have orthotics, and first thought what was going on was plantar faciatis, however now… I think I’ve figured out what area is paining me.
My abductor hallucis muscle aches ALL the time. I can kinda just feel it, and lately, I’ve been having stabbing pains from my heel/ankle toward my big toe. I understand there is a nerve there, but I don’t really know what could be going wrong, as this seems to be a unusual problem. This is really affecting my running and now kinda my cycling, as my right foot is very sore after runs, or any activity that I am on my feet a lot.
I am 6’0, 177, high arches (they don’t touch the ground in the middle) and flexible feet.

One of the tibial nerves runs around there, IIRC. Do you pronate a lot? My first suspicion would be a fracture of the metatarsal that forms the pad at the base of your big toe, or inadequate arch supports.
I’ve got pretty much the same thing, daily soreness behind my first metatarsal, esp. after a run or bike and sometimes on waking up in the morning. Usually not sore at all while I’m running or biking. Also get the tingly ‘needle prick’ pain in my arch quite often while running or just sitting. I’ve also got high arches. I’ve found going at my arch with the TP wheel nightly seems to help and at least keep things at the point where I can still run and bike a lot without needing any real downtime.
Tarsal Tunnel Syndrome seems like it, pretty much. The problem being that I already have orthotics for running… so maybe I should get even more arch support… or is it possible to pronate when cycling? I don’t have cycling orthotic inserts and have been doing more cycling than anything lately.
I do pronate with my right foot, and have this weird issue where my hip won’t stay in quite the right place so I have to get a chiro to pull it back every once in a while. I end up with my right leg being shorter and ITBS type symptoms at my hip joint. I don’t think they are connected I just mention it because my right leg has all my chronic injuries.
Actually, they ARE connected (both literally and pathologically)
Your shorter leg will tend to cause more pronation at the subtalar joint to functionally “lengthen”. This STJ pronation stresses the medial ankle, PT tendon, tarsal tunnel, etc. and can be a major cause of tarsal tunnel syndrome (especially if coupled with a more high arched foot - yes, a high arched foot can still pronate a lot).
Definitive diagnosis needs a thorough clinical exam and possibly even an NCV (Nerve conduction velocity) test - NOT a very fun test (involving lots of needles and electricity) ;(
