I carry my insulin pen on my forearm while running…I wear an arm warmer/cooler and slide it inside. You might also try a velcro Road ID to hold it on your forearm, I’ve never tried that for my insulin, but it works pretty good for carrying a gel packet.
don’t have to carry one myself, but SPIbelt has an 8.9 inch belt that fits the epipen.
When I need to carry something I use the Nathan Shadow pak (they make a weather resistant one now too).

How big is it? I want to say carry it on your forearm like some runners with a huge MP3 player or a cell phone. There are wallets and music player cases with a forearm strap. See if the pen will fit inside those.

I have been using the SPI Belt for the last two years on all my runs. At races, I use it as my racenumber belt, and put it on before the bike ride.
jsut get an elastic band around the forearm or bicep and slip the pen in there. voila.
+1 on the Spibelt. My sons both have them to carry inhalers and epipens.
