2 months ago I had the Boulder Center of Sport Medicine do a fitting for me. Their fit philosophy is a bit different than you will find from this Forum. They tend to fit for comfort and power production, not so much Aerodynamics.
I have not been able to ride much outdoors in the position they put me in, so I cannot provide any quanitative opinions. They are very passionate about their method.
They have both a physical therapist and a bike fit specialist work with you using a 3d camera and computer analysis to determine the proper fit. They are not cheap, ±$400, but if you can get a presrption from your DR. your insurance may pick up the tab since it is a physical therapy / hospital.
