Survey - Cycling Awareness and Safety

One of my colleagues here at Indiana University is studying how cyclists and drivers interact on the road, and how to design systems to improve cyclists’ safety. Here is the statement of purpose from his abstract:

"The purpose of this survey is to understand travelers’ attitudes and behaviors towards the other road users. The goal is to design ways in which travelers can be more aware of each other’s intentions when sharing routes and several forms of conveyance, including pedestrian, cyclist, velomobilist, drivers, and associated vehicle types. The project is part of the Sustainable Interaction Design Research Group (SIDRG)’s activities. "

The survey caught my eye as something that perhaps the Slowtwitch community may be interested in giving input on (it is a topic I’m passionate about as well), and so I have gained permission from him to post the link to his survey on this forum. Here is the survey:
