i run about 25 miles a week on road surfaces and just started having right inner/lower knee pain. Left leg is fine. I’ve never had any knee issues in the past. I’ve been training for about 10 years. I started using the new Hoka’s for most of my runs. I used Pearl Izumi project EM all last season. I also use Superfeet green inserts.
The only thing I see is an imbalanced wear pattern. Right outer heal is wearing quick on the right shoe as well as a slight difference on the inside of the right shoe up near the big toe knuckle.
Anyone know what’s going on?
Here is what the wear looks like:

You can’t discern anything meaningful from shoe wear. There’s a good chance nothing is structurally wrong with your knee. I’d see if you’ve got any trigger points/knot/adhesions in your calves or quads. Tightness there can cause the knee to track incorrectly - among a number of other things that manifest as knee pain. The root is likely in either your quads or calves.
I’d recommend picking this book up - you’ll get more out of it than 95% of visits to pt’s and docs who don’t specialize in runners:
