Opposite side of the coin, offensive?

I’ve got some paperwork I need to fill out that requires people’s gender. Ninety percent are obvious but one person, I’m not sure how they identify. But I don’t want to ask because I’m sure some people would be offended by the question. She’s obviously a female, but at some point there was some “they, them” pronoun usage being talked about. But that seems to never have materialized and everyone just uses female pronouns for her. I checked her email signature and no pronouns are listed.

So, just go with the sex or ask about gender?

What’s it for? Meaning how “binding” or serious is this form? Is it something where “shit I really had no idea, I apologize” is acceptable if you just fill out the form as you see it?

Are there more than two options for gender?

SNL used to run a skit with a character called “Pat”.

Everything revolved around interactions with Pat to determine if s/he was male or female.

I’m not sure that skit would fly today?
