Both of my big toes are a bit swollen, red and sensitive to contact around the nail.
No itch, doesn’t bother me walking around. Only if I bump the top side around the nail.
Nails are maybe starting to get a yellow hue.
I can run, but my perfect fit run shoes are now just a bit too snug at the tip of the big toes due to the swelling.

The only definitive way is via nail unit culture for potassium hydroxide prep microscopy (KOH prep) or periodic acid shiff (PAS) stain - then culture. Generally a clinical diagnosis though. By the time tenderness around the nail ensues though, it is usually VERY obvious (which yours does not sound like) - sounds more like a chronic subungual hematoma.

It does not look like this.
No pooling or bruise under the nail that I can see on my feet.
My toe flesh is redder, looks kinda like it’s plumped up everywhere fwd of the knuckle, enough so the nail looks sunken in.
More like this, absent the blood. Sorta makes sense as I recently started to cut my nails back a bit shorter than usual, at least that’s my latest theory, which will change shortly I’m sure.
