What’s the little hook or lever on his aero bar right extension?
It’s a brake lever

Dumb question- for the front or rear? How does it get cabled in with the other brake lever?
It’s for the rear. It wouldn’t make sense to have the front brake on the left side and then switching it sides for the little lever. The cables are split, it’s a system common in cyclocross bikes.

Its funny to see that on a euro pro’s bike. Aerobar brake levers used to be common in the pre-Scott days when RAAM guys and others used to make aerobars out of plywood or fabricate them themselves.
There’s no splitter. He is running the lever in series with the other “normal” lever. Routing like so:
Cable housing from frame to left hand aero extension. Loops out from end of extension (at 90 deg. to extension) across to r hand extension. housing runs to lever and terminates. New housing length from extra lever to standard lever. Housing terminates at lever.
“extra” lever functions by changing length of housing run, causing brake cable to move, causes brake to operate.
