Foot discomfort inside of foot under ankle next to heel

You’ll have to imagine this as a man’s foot as this was the only picture I could find in google images to point out where the problem was…

I’m not sure how to describe this but here goes. pain scale of .75 on a 1-10 but something don’t feel right. If I put my foot flat on the floor then pick my toes up I feel this twinge/click that’s on the spot in the picture. I can continue to raise and lower my toes and not feel it again. If I lift my heel with my toes on the floor then put it back down and lift my toes there’s the click. I’ve had it about a week and figure its the result of a long trail run I did a week ago I did the same run today 13 miles and it didn’t appear to aggravate it worse than it was previous…thoughts? I don’t think its faciatis because its on the side of the foot not the back or underneath.

did it come on right after the run?
Is it immediately under, or just on the back side of the malleolus? (The tip of that ankle bone above the arrow.)
Is it tender to touch?

Where is the rest of that picture?

LOL! Pretty funny.

You are likely feeling the flexor hallucis longus tendon binding down as it runs in the tarsal tunnel. You may also have an os trigonum (small accessory bone behind the ankle at the level of the talus) that can also predispose to this.

Try the same “experiment” you mentioned, foot flat, but raise your great toe only. Also, plantar flex your foot as much as you can (point your toes like swimming), then dorsiflex your hallux and up and down. Feel anything?
