Dislocated Ankle

During running intervals, I dislocated my right ankle. After 10 days I got it readjusted. The good news is that I can roll over my heal again. However, the area under the inner ankle bone is still pretty painful. The pain also radiates into the area about 4 inches above the ankle. The x-ray didn’t reveal any fracture. If a tendon might have gotten severely stretched or is even partially torn is unknown. Even though I’m able to cycle again, pushing into a headwind or climbing hills is a “no-no” - and so is running. Any suggestions/recommendations on how to treat it and how long the recovery might take? Thanks.

99% ankle injuries do NOT show on xray.
If you are running on it already, you did not “dislocate”

will take months and months to feel normal, even if “just” sprained.

Need a thorough exam (possibly advanced imaging) to assess for deltoid ligament, spring ligament, and or posterior tibial tendon injury (to name a few). It is “estimated” that up to 70% of ankle “sprains” have more advanced soft tissue damage then a simple ligament sprain. These don’t necessarily all need “treatment”, but many do.

What the other guys said. Listen to everything that rroof says btw before the rest of us.

However, sounds like you had a traditional ankle sprain. My best advice, from someone who has sprained their ankle more times than I can count and is now missing his anterior tib-fib ligament, is to see a good PT and follow through on your ankle rehab and strengthening. If your insurance will cover it, a CT or MRI will give you a better diagnosis on any tears.

Do some pool running, focus on your swimming ( go easy on the kicking and flip turns) and strength training for a few weeks, use pain as your guide if pushing on the bike. Incorporate some more walking into your days before introducing running. Give it some time to heal properly.

Unfortunately, I’m still far away from running. The joint was put back into the socket 10 days after the injury occurred, which enabled me to at least walk again. :slight_smile:

Not sure if I sprained it. I never twisted the ankle or stumbled over anything. But once the joint was “back in place”, I felt a lot of instability - as if there was a big void.

I have been water running (even though it’s very boring :slight_smile: ) and swimming with, as you said, taking it easy on pushing off the wall. I was trying to only kick the other day, which is still not a good idea.

I am also doing lots of strenghtening. I started with simple towel scrunches and picking up marbles, and have slowly advanced to toe raises.

Sorry to hear about your anterior tib-fib ligament tear. How did that happen? Did you have surgery? Are you back to running/cycling? Can you compete as you have before?

Luckily, I have a pretty good PT I trust. He has put me back together quite often.

BTW, I remember rroof from when he guided me through as I had broken my navicular on my other foot and was faced with surgery including what type of bone graft to use. He was also instrumental in getting me back on my feet after 5 long months on crutches.

Thanks rroof. You may not remember me, but you were instrumental in getting me back into triathlin after breaking my navicular on the other foot in 2006. I also bought a bone stimulator from you back then. I’m happy to report that that foot has been strong as can be - even with the bone graft and screws. :slight_smile:

Could the tendons you are referring to been stuck on a bone when the joint got dislocated? That’s what it felt like.

How long might it take to fully recover and be able to run again and compete?

Also, what treatment would you recommend?

And last but not least, if the joint was indeed initially dislocated, will I be prone to that happening again?

Way to difficult to answer. Oddly enough, I was actually at an ankle reconstruction cadaver lab training at Arthrex headquarters in Naples when I read this. Some of the best sports med guys in the country there and we had a great time (except the Clemson team doc getting jabbed at by some Alabama guys) :wink:

Lateral ankle “sprains” and issues are far more common and are typically easier to deal with. Medial ankle injuries/issues are much more complex as the acute phase can take a lot longer to recover from and some chronic issues can ensue as well. They are also usually far more complex surgeries if that is ever necessary. New procedure I literally just “learned” yesterday to deal with a particularly tough issue. Can’t comment on any treatment though without a diagnosis. Hopefully just a grade I injury and you will be good to go in a few weeks. Utilize your PT that you trust for sure as he/she can guide you and also suggest when to possibly seek more help.

Ok, good- sounds like you are in good hands! My ligament apparently totally ruptured after too many sprains/tears over the years- had some ultrasound scans done to diagnose but it had been like that for a long time so I just left it as is. It’s fine and non-symptomatic, however I can turn my ankle fairly easily so I just stay off the trails. I do feel like I have a bit of arthritis in that joint at times…
