Diarrhea on the run: Help!

Without disclosing any names I’d like to solicit this forum’s advice.

I have a training partner who frequently develops GI issues on long runs when exerting himself at below LT threshold level at, say, PE 3 +, for prolonged periods. For instance, this person always has bowel cramping and diarrhea during the run portion of HIMs. Having personally reviewed this person’s Garmin data I noticed this condition coincides with an elevated HR. Consequently, I believe it may be due to blood shunting in the gut. (That said, I am not a doctor so who really knows?) As a precaution, this person modifies his diet for several days prior to an endurance event, i.e., reduced intake, elimination of dairy products, etc., but has failed to achieve any relief.

Next step is to consult a GI internist; however, it’s difficult to find a such a specialist who is also an endurance athlete. I am certain this can be fixed. The question is how to go about it.

Kindly respond with constructive suggestions only. No jokes, please. This is a serious matter. Many thanks.

This person could either try hard to flush out the system before hand. There are a variety of ways to do that.

Or, try immodium.

eliminate fiber and write down all nutritional intake. Switch nutritional habits and maybe they’ll notice what contributes to the issue. Good luck. Nothing worse than a shitty run.

Good replies both. Thank you. If anyone else would care to share their experience or offer some advice I would be very grateful.

I have the same problem. If you solve it, please let me know.

For sure stay away from Gatorade. Heed works better, I have not tried Infinit.
I have this problem on shorter runs, where no nutrition is needed though too. Gatorade just amplifies the problem.

I’ve tried the GI doc, fasting, flushing, immodium and just about anything else out there. NO LUCK.

Do you have a higher than normal HR when you run? (Am trying to determine if blood shunting in the stomach is the issue.)

I have found protein really helps. If I increase my protein intake and reduce fiber the day before and take in nutrition with protein during my run I avoid a lot of issues. I still haven’t gotten it exactly right but I’m making progress.

Yes. a very high heart rate.

In fact I quit using a heart rate monitor on the run because it was psyching me out.
I can run 8 miles with my heart rate above 188. And that’s at a 10 minute mile pace.
So my heart rate should not be at 188.

When my heart rate is 165 I consider that my Zone 2.

I have a feeling it has something to do with the blood as well. Just haven’t been to the
right doc to get it figured out.

Thanks, Jen. A little protein is a new twist I’ll have my friend try.

Hey L,

I think you have the exact same issue.

It’s funny, while running with my training partner, I always question his personal characterization of his own PE during workouts. This is because his HR and respiration seem pretty high to me relative to his self assessment on a Friel scale’s PE.

Another query:

When you’re doing your 10 min/mile pace @ 188 bpm, will someone who doesn’t know you well perceive your breathing to be somewhat labored if you are conversing during the run?

C

During that particular run, which is the run which made me stop wearing the heart rate monitor, I did feel like my breathing was labored.
Although I did feel like I could have kept running.

Like my training partner, I think your natural physiology mismatches your muscular stamina with a slightly “underpowered” cardiovascular system. It’s very interesting how both of you experience identical symptoms when physically stressed. I think we may be on to something.

I have put my increased heart rate together with increased stops to the toilet on the run. I just haven’t figured out what to do about it.
Like I said above, I have tried just about everything including prescription anti-diarrheal and haven’t found the magic recipe yet.

One thing I know for sure 5 or 6 stops at the bathroom during a marathon will hurt your marathon time!

Thanks Lori. I will keep you posted about what else we learn. And thank you for your candor. Chip

I combine pepto with immodium, havent had any problems.
