I’ve been really happy with Virklon (http://www.virklon.com). They are out of Spain. Turnaround time is about a month - about 3 weeks before they produce your clothes, plus shipping time (I’m in the Midwestern U.S.). Prices are reasonable, especially for custom stuff, there are no minimum orders, no upfront design charge (unless you want them to help you design your clothes - you’ll need some vector graphics skills), they have a really nice selection of clothing pieces for triathlon, cycling, running, and kids, and they are super helpful.

Sizing is maybe just a bit on the small side, so if you’re kind of between sizes, you might want to consider going bigger. The clothes are comfortable and seem to be well-made. I’ve worn the aero tri top/shorts in a couple of races and find the chamois to be really comfortable. The road kit has been great, too. A few of my customers have worn my kit or designed their own and have also liked the fit, comfort, quality, and materials. The “breathe” tri tops are kind of see-through, so you might want to consider that.
The clothes and order page are designed to support a “your name” on the clothes. If you leave this out of your design, they’ll kind of wing it, which might not be what you want.
Pricing is in Euros, but I think pretty reasonable. Shipping to Indiana is 11 Euros per order (at least for the sizes of orders I’ve placed), which isn’t too bad.

Edit: I forgot one thing - their clothes have their “intelligent retention system”, or IRS. It connects the shorts to the tops. I like it - it keeps my tri top from riding up on me while on the bike so I don’t get a tramp stamp sunburn.
+1 for Virklon, I’m a fan. I needed an option for a custom sleeved tri top because my club shop don’t have that, and these guys came up trumps. The prices are very reasonable, and you can order as many/few as you like. Not as flash as Travis’ team kit, but I came up with a design that came out nice with no prior vector graphics experience. I downloaded Inkscape to do the design.
