Chronic Glute pain for 3 years 15 year old boy

Wondering if someone is able to assist, my 15 year old boy has chronic glute pain for 3 years off and on.

He is unable to be active when the pain occurs. we have seen Osteos, Chiros, Physios, sports medince docs and even a sports Pshc. Conducted MRI’s, X rays, and Ultra sounds which all have come back clear.

Different diagnoses however nothing has worked, from hamstring, SI joint and Hips. We are in the process of strengthening the core and lower body.

The kid is in tears as he is a talented sports kid, however cannot perform.

Is the pain more located in by the tailbone or more in the musculature? Also, what type of pain, burning, aching, tingling, and also and radiating symptoms down the leg?

It’s an aching pain in the middle of the buttocks.

At times pain is also in the lower back region. No pain down the leg.

The location would point me towards an issue with his piriformis, if he had radiating tingling it would even more. I would try using a baseball, softball, or lacrosse ball and trying to roll it with that, right center in the glute and roll around until he finds the most tender spot and then focus on there.

If he had any pain around the side and into the front/groin I would look at the labrum. MRI might not show that if it was done without contrast.

We have tried the ball with no great success. We will starting it again

Got it, that was just my first thought based on the symptoms from afar, lol. The other option, if you haven’t tried it yet, and again this might depend on where you are located, would be to try dry needling. In my experience if you aren’t able to get to it with rolling or with a manual piriformis release then my next step would be to try dry needling. Have had good success with my former athletes dry needling.

thanks heaps - dry needling to the glute or the SI joint?

I would try to hit the piriformis in an attempt to get it to release, but would also do a couple more around it to target some nerves but you could also get some done focusing on the SI joint while you are there.
