Not everyone that runs Boston has to qualify. There are thousands of charity runners. Fewer than NYC, but still a few thousand.
You seem to have overlooked the last line of my statement that read:
“The obvious exceptions, in both cases, are the charity/celebrity/freakishly wealthy entrants that represent a minority of the overall field.”
