Take it all apart. Clean everything. Add some custom “knurling” with a file if there are parts that have been worn smooth. Use Loctite. There are several kinds that will keep things tight but not completely locked in place.

A shop guy told me this week that Look will have some diamond encrusted washers in the spring to deal with the slippage problem.
We have three of these stems and find that they will work if you’re really careful about how you tighten them.

Can you please share what you mean by really careful about how you tighten them?
how about trying the rough feeling carbon assembly paste from FSA?
Mine slipped initially, so I searched for the instructions on the Web. The bike shop didn’t give me the instructions or tell me how I was meant to do it which was really annoying.
You actually need to tighten both bolts gradually. (this is A-head stem instructions but I assume the quill stem ones are the same)
If you need to change your position a little, you really need to loosen both bolts and then tighten them both up gradually. Going from one to the other and making them tighter and tighter.
Also - you need a really long allen key, or some extra leverage on a normal allen key (a big metal tube) to get enough torque.
Just tightening by hand with a normal Allen key will not be enough.
Test it before you ride on it by grabbing your handlebars and trying to wrench then up or push them down. Will save you getting the crap scared out of you when they slip whilst the are descending at speed!
