Zipp 145 Stem Loose on Fork?

I recently dropped off my Zipp 2001 to replace the drivetrain/aerobars and stem but the LBS has run into an issue. Apparently the Zipp 145 stem with shim won't clamp securly to the Cervelo carbon fork (late 90's model?) with a 1'' steerer at the recommended torques. The question is whether the fork is undersized or the shim is undersized. Aside from that, do does the stem need to be torqued higher than the recommended values for a proper fit? I'm hestitate to tell them to tighten it beyond recommended values, considering the posts I've seen with cracked carbon handle bars and stems from over-tightening.

One option the LBS offered was the knurl the steerer to increase the effective diameter, but that seems like a last resort. Other than that, should I simply replace the fork?
