Mono bar extension failure [Factor Slick in this case]

Just looked at this video about 10 times, and I'm not sure what the failure is. Could be bad design, could be poor installation, could be bad luck.

For my question, it doesn't really matter, what the cause is, but I'm wondering whether a normal type of extension (two seperate extensions) is safer.... In case 1 fails, you might just be able to hold it (alltough falling would be an option with 1 failure as well).

Seems like an interesting debate, given the fact more bikes seem to come with mono extensions (p5X, Trek SC, Factor Slick, etc.).

guessing that it's just a case of the "once over" not being thorough. That's where I would start looking for sure.

Wow, not sure if having two extensions would help you at that speed! Squeaky bum time!

I had a zipp vuka alumina arm rest snap off at mile 49 of a 50 mile TT, fortunately I was only going about 38kph and managed to stay upright and finish.
