Help diagnose a foot issue

In May I developed a pain in my left foot. Over a 3-4 week period, it went from a dull pressure to a sharp pain that I eventually couldn't walk on without wearing a donut pad. It felt like I was stepping on a piece of my kids lego with every step.
I was running a few times a week and a little MTB at the time this developed, no major training.

My doctor thought it was a callus... I used callus pads to kill the skin in the area, that didn't help.

I went to a Podiatrist... he said it was a cyst and surgery would be required... see pictures. He cut out what you see in the picture. It was solid... not something that could be drained.

The post surgery pic was one day after... it took about 4 weeks to heal and I slowly got back into running without any issues.

3.5 months after surgery, it's coming back. The pain has been noticeable for the past week. I have been running ~60k a week for the past 6 weeks.
I have circled the area of concern in one pic and then the 3 specific areas where I can feel small lumps that are sensitive to touch.

It isn't at the point where it is preventing me from running yet but it's getting worse.

Is it really a cyst like the Podiatrist thought? Why is it growing back? What can I do to deal with this?

I'm not a doctor nor do I have any interest in diagnosing anything.

Have you been back to the podiatrist? As frustrating as it is I'm sure, it seems like you should at least go back to the podiatrist. Maybe the way the pain came back rules something out but if your doc doesn't get your feedback he can't finish the follow up. And then my last idea: Was a plantar wart ever considered? It kind of looks like one.

are the photos gross? Need to know how gory before I decide if I am willing to look.
is rroof still around? He is the type of doc you want - foot specialist. He is good people!

I am waiting on a call back from the Podiatrist that cut it for his thoughts on why it is back... He is a small town Podiatrist, he made no indication that he had seen this before and my confidence in him is lacking.
I am hoping to hear from someone with some more experience.

Not a doc and I'd defer to your podiatrist. That said, what you describe is exactly what my daughter went through with plantars warts. They were surgically removed and her doc said they might come back.

agree about rroof, his two cents is worth a dollar.

doesn't seem like a cyst if it's hard/granular stuff, plantar wart more likely.
the underlying cause needs to be treated (HPV) as they will just come back.
