Gross Hematuria from Cycling?

I've been doing a lot of cycling (for me) this year and only just started introducing a few shorts runs - I'm taking kids away for an 8 day holiday next week and figured doing some running whilst away will offset cycling fitness loss. I ran Mon-Wed-Fri nights at a decent tempo (given I have not really run for months).

Yesterday (Sat) I went for a solid tempo ride; approx 75km at 270 w NP. When I got home I peed and felt a sharp pain first up and noted what looked like a blood clot shooting out. After that there was no pain and pee 'looked' clear. That was about 6pm.

I had dinner and watched some tv. Drank about two glasses of water and at 11pm went to pee before bed. Again another long, thin 'worm-like' clot shot out with a little bit of pain then clear pee. This time at the end fresh blood was dripping out. I drank heaps more water then went to bed. No clot or other signs of blood at 4am nor just now.

THough I will make an appointment this week I consulted Dr Google and came up with exercise induced gross hematuria, but most of the articles link it to running. I have been guilty of being very lax with hydration on pretty much all outdoor rides (yesterday I probably had 500ml max in the 2:15 hr ride).

Just wondered whether anyone had experienced something like this from what has really been cycling only. I dont think the little 5km, 22 min runs could have done it.

I have no idea what you have but I winced multiple times and had to put my hand over my face out of shock.

Feels like an ER visit maybe?!?!?!

I had this happen as well.
20 miles into a bike ride I stopped to use the room. Peed blood with some chunks in it. No pain, told a buddy about it 5 miles later. He said go home and go to Dr.
Road home, peed a little blood, less chunks no pain. Made a dr visit either that day or next day ( I am sorry this was about 10 yrs ago). By then pee was normal. Dr said it happens sometimes and was normal.
It freaked me out.

So hit your Dr up.
But do not freakout yet.

Not a doctor here but I do work in the medical field and do diagnostic imaging. Gross hematuria, first thing I would think of is kidney stones, especially if you have flank pain. I'd ask for a CT Urogram. Good luck.

As the poster above me mentioned, it could be kidney stones, I am dealing with one right now. Recently when I run, it "shakes things up" and I can urinate a blood clot. No problems when I cycle though. Usually I get terrible lower back pain but from what I read if it is a smaller stone you may not feel much pain.

I went through exactly what you described and after a lot of tests and procedures, some painful, I have been diagnosed with prostate cancer. Gleason score of 7 so pretty serious according to doctors. Absolutely see Doctor. Hope it is just exercise induced and not something more!!

Hemeturia could be a lot of things or nothing at all. It ain't normal. Most of the things other than exercise stress are not all that good. Kidney stones, bladder stones, prostate cancer, BPH, bladder cancer, Ureteral cancer, and renal cell carcinoma are a few of the more common ones. I would make an appointment with my local Urologist and see what he/she says. They know some stuff that the folks on the forum don't. Had a friend that committed suicide by procrastination. He peed blood for six months then went to the Urologist. I told him to get his arse in there asap, but no. Come to find out, the bladder cancer got both ureters and kidneys in the time he delayed. Then it was not a pretty way to go. My advice is don't &#$@ with hematuria. Go see the Urologist.

Gave four vials of blood and a urine sample this morning. Will see what comes from it.

All the best, I have had this from running twice (note I am a doctor/specialist). Both times running I was probably running with what I would say is a completely empty bladder and I was pretty dehydrated in the heat. It has not happened outside of these situations. Kidney stones would generally give you pain first and foremost. Bladder cancer can be hard to pick, but you would expect there to be blood outside of exercise times. Prostate cancer would be rare in terms of presenting with haematuria. Not as easy to explain why it happens cycling rather than running (where there is more chance of bladder trauma).
You sound like you are doing the right things...be good to hear the outcome when you know more.

So far the results have come back largely within the cited ranges:
Renal function 89
Glucose Rand 4.5
Full blood count parameters all within acceptable ranges
Cholesterol and triglycerides below the 5.0 and 2.0 limits (4.6 and 1.1)
Liver function test generally within the ranges except ALT with the comment about mild hepatitic pattern commonly seen in medications, fatty liver or iron overload
Coagulation study within range
C reactive protein <1
Uric acid within range

Will look to see a urologist as a follow up.

There’s been no flanking back pain. I’ve had dull back pain for a while but that flares up when I sit poorly in bed on lap top.

That is good news. I would not be too worried re the liver enzymes being slightly off.
Just do be aware that a urologist will almost certainly want to do further investigations which may or may not really be warranted at this time (where are you located?). We have a saying in medicine that if you are holding a hammer most things look like a nail, so see a surgeon and they almost always want to do a procedure of some description.
Not saying don't get it checked, but just be aware often their first step is either more imaging or a small op to look inside etc (OK, they can do this with you awake but having a snake shoved up your penis ain't no fun at the best of times).
