AC Joint Attenuation

After a crash at mile 107 at IM Texas this year, I've been having issues with my right shoulder. A June MRI showed "moderate to severe AC Joint attenuation and arthritis". I've been seeing a physical therapist, with little to no improvement. About 4 weeks ago, we thought we had this thing whipped enough to resume swimming - that was not fun. I could not complete a 400 m warmup without pain, so pulled the plug.

Have any of you had this diagnosis before? What was your course of treatment? Did it get "fixed," or is there always some low level pain I'll just have to deal with?

I'm a 30 YO former swimmer, so shoulder issues shouldn't be a surprise, but the arthritis did shock me. I'm not averse to or scared of living and working through pain, just want to get an idea of what expectations I could or should have.
