simple knee scope turned into a nightmare- what to do now?
5 months ago I had my knee scoped. They repaired the lateral meniscus , removed the plica and chondroplasty. The MRI showed some ACL damage which I had repaired 20 years ago. When he did surgery he said "the ACL wasn't that bad." It had minor stretching and fraying to the ACL. Also, the doc said I had normal wear of a 40 year old and the meniscus was a small fray.

About a week after surgery I called in to report a sharp pain, like a hot poker was stabbing me. That was the only pain I experienced after surgery and thought it was just healing. I experienced this few more times over the course of a few months.

It took me 10 weeks to walk normal due to stiffness, but no pain. I was riding my bike right after surgery per docs orders and no pain with that. I eventually got back to power walking with some light jogs and worked my way up to 5 miles. I did therapy exercises religiously . The doc also got me an e-stim unit for my quad muscle which I used daily. About 4 weeks ago I finished up a power walk and my knee has been the worse it ever has been. I've been back to the surgeon twice. Ten days ago he did cortisone and that has not helped. Some days seem okay and other days, like today I can't make it out of work w out a crutch.

I'm going to a new PT tmrw, but I'm worried that just the straight leg exercises/quad tightening cause it to flair up the next day. I also went to another doc in another hospital on Friday. He said 2-3 months of new therapy and if that didn't work he would send me to someone different. My leg was is really weak and not sure how to get the strength back because I think just the easiest things cause issues the next day. I had x rays and spacing is normal. I asked about another MRI and he said he wouldn't suggest this at this time.

It seems like after going in for a quick clean up turned into a nightmare. I'm not sure where to go because I'm afraid the easy exercises cause issues unless there is another problem.

Has anyone been in this situation before? Do I see another doc until they give me another MRI? I just want to be able to walk normal and eventually be able to bike.

I work as a physiotherapist (physical therapist).I would assume that you have addressed not only your quads strength, but probably more importantly your hip and pelvic control?

If this is the case my suggest is to see someone who specialises in ‘how you are moving’. By this I mean what is your walking, standing and sit to stand movement pattern like. You may be irritating your patella femoral joint mildly, but regularly if your movement pattern is poor! This would create the symptoms you are describing.

I've had 4 (separate) meniscus clean ups and an ACL (with meniscus clean up) and never had a sharp pain or really any residual issues even pretty close to the surgery. My honest guess is your surgeon didn't do a good job and there's still damage in there. I'd go to a new surgeon and get it fixed for real this time.

I’m going to a new pt today. I’ve been thru a ton of therapy already so we will see. I’ve had my gait evaluated and I’m cautious about my sitting posture and even do stretching during the day. It seems the little pt exercises make it flair the next day. All I did the night before was leg straightening and quad tightening. Hopefully someone can pick up on something. I haven’t rode my bike in a month and getting desperate

I had meniscus surgery done in Ohio at the end of 2016. Pretty much can’t run now because they said all of the cartilage was worn away when repaired. I still have pain when my leg bends a certain way. Had to switch to just cycling, which I like but wish it could have been running...

Sorry to hear that. After surgery my doc made it sound like it was a minor repair and normal wear for my age. Now it’s become a disaster

I hope you can get addressed through PT!

I had a bilateral scope surgery 25 years ago and ended up in a long term follow up population. They call me every 5 years and the questions always freak me out. Can you walk up less than 1 flight of stairs, 1 flight of stairs or more than one flight of stairs? Are you comfortable no walking, walking to the mailbox, around the block, no discomfort issues. That said I just had my best mogul skiing season of my life - but man it freaks me out when they read me those questions.

I too had my running career ended by a meniscus tear; caused by hyper extending the knee when I pushed off the wall in the pool. That was a long time ago and I haven't run since but thankfully I can do pretty much everything else I want to do without any pain. One thing I have learned in my quest to stay mobile is that the knees are at the mercy of the hips. Keep your hips mobile, strong and aligned and the knee issues are much better.

I think the OP should maximize the PT program before thinking about surgery again. If the PT is not effective after a year then I would start with a different doctor in a different city. Too many of these guys know each other socially and are hesitant to tip the apple cart. Find a Dr that you think can be objective and start over if you have to. Hope it doesn't come to that - good luck!
